52. AMENDMENT TO HEALTHY START LEASE AGREEMENT L-7471 Approve and execute Amendment No. 1 to Lease Agreement No. L-7471 with Eastern-Columbia Properties, LP (Lessor), for 5,230 square feet of rentable square feet of office and clinic space located at 3230 East Broadway Road, Suite A200, Phoenix, AZ 85040 (the “Premises”). County has this one option to renew the lease for three years by mutual agreement. The term of the Amendment will commence November 1, 2020 and expire December 31, 2023 at rate of $13.52 per square foot, a reduction of $0.92 per square foot, plus rental tax, and, expand the space from 5,320 square feet to 7,776 square feet. Lessee may “hold over” for up to three (3) months at the then current lease rate and on the same provisions and conditions in effect at that time. The lease may be terminated by mutual agreement upon one hundred eighty (180) days written notice one party to the other, pursuant to A.R.S. §38-511 or for non-appropriation of funds. Lease payments will be $8760.96 a month ($105,131.52 annually) over a 38 month lease term. (C-86-18-020-1-03)
