TITLE VI ASSURANCES AGREEMENT WITH THE STATE OF ARIZONA DEPARTMENT OF TRANSPORTATION FOR SAFE ROUTES TO SCHOOL PROGRAM Approve the Title VI Non-Discrimination Assurances between the Arizona Department of Transportation (ADOT) and Maricopa County by and through its Department of Public Health (MCDPH) as part of the FFY21-22 “Safe Routes to School,” self-administration application approval process through ADOT. The Title VI assurances is a standard U.S. Department of Transportation form and therefore not all appendices will apply to the SRTS program due to it being a Non-Infrastructure project. The Title VI form is required to be renewed every year to remain valid and in compliance with ADOT requirements. (C-86-18-031-3-02)
