ACCEPTANCE AND USE OF REDEEMABLE VOUCHERS FOR THE TB CONTROL INCENTIVE PROGRAM Approve the acceptance and use of 200 Walmart vouchers provided by Arizona Department of Health Services (ADHS) to Maricopa County through its Department of Public Health (MCDPH), TB Clinic (TB), TB Control-Incentive Program. The MCDPH TB Clinic will receive a donation of 200 Walmart vouchers valued at $10 each for a total value of $2,000. In addition, please approve the acceptance and use of additional vouchers, not yet requested, and estimated to be valued at a not-to-exceed amount of $3,000. Total approval requested is in the amount of $5,000. MCDPH’s indirect rate for FY2017-18 is 16.6%. This in-kind donation does not allow for indirect; therefore, indirect expense estimated at $830 is unrecoverable. (C-86-18-031-D-00)

ACCEPT AND USE OF REDEEMABLE VOUCHERS FOR THE TUBERCULOSIS CONTROL INCENTIVE PROGRAM Approve the acceptance and use of 240 Walmart vouchers provided by Arizona Department of Health Services (ADHS) to Maricopa County through its Department of Public Health (MCDPH), TB Clinic (TB),TB Control-Incentive Program. The MCDPH TB Clinic will receive a donation of 240 Walmart vouchers valued at $25 each for a total value of $6,000. In addition, please approve the acceptance and use of a $1000 voucher to be used for the TB Program only. Total approval requested is in the amount of $7,000. MCDPH’s indirect rate for FY2017-18 is 16.6%. This in-kind donation does not allow for indirect; therefore, indirect expense estimated at $1,162.00 is unrecoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This donation does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this donation through a future reconciliation. Funding for this Agreement is provided by a Donation from ADHS and the American Lung Association of Arizona and will not affect the County’s general fund. (C-86-18-031-D-01)
