CONFIDENTIALITY STATEMENT AND COMPUTER SYSTEMS ACCEPTABLE USAGE AGREEMENT WITH HONORHEALTH FOR CONFIDENTIAL PATIENT IDENTIFIABLE INFORMATION Approve non-financial Confidential Statement and Computer Systems Acceptable Usage Agreement between HonorHealth (HH) and Maricopa County by and through its Department of Public (MCDPH), Disease Control Division to provide access to certain confidential patient identifiable information available electronically. HH will allow MCDPH access, and to permit its physicians, healthcare providers, and employees to access patient information to provide patient care and disease control services. This agreement shall be effective upon signature by all parties. The Disease Control Division routinely request and receive medical records from hospitals and healthcare providers in the course of disease investigations mandated under the AZ Administrative Code Title 9, Chapter 6. (C-86-18-033-M-00)
