DATA USE AGREEMENT WITH CHILDREN’S HOSPITAL CORPORATION D/B/A BOSTON CHILDREN’S HOSPITAL Approve a non-financial Data Use Agreement (Agreement) for Research between Children’s Hospital Corporation d/b/a Boston Children’s Hospital and Maricopa County by and through its Department of Public Health (MCDPH), Disease Control, for access to anonymized data collected by Boston Children’s Hospital. MCDPH agrees to use the data for local public health surveillance and as defined in Appendix A of this Agreement. This agreement shall be effective upon signature by all parties and shall remain in effect for one (1) year, unless terminated earlier with 30 day written notice. (C-86-18-040-3-00)

AMENDMENT TO DATA USE AGREEMENT WITH CHILDREN’S HOSPITAL CORPORATION D/B/A BOSTON CHILDREN’S HOSPITAL Approve Amendment No. 1 to non-financial Data Use Agreement (Agreement) for Research between Children’s Hospital Corporation d/b/a Boston Children’s Hospital (BCH) and Maricopa County by and through its Department of Public Health (MCDPH), Disease Control, will extend this Agreement for two additional years and provide for access to anonymized data collected by Boston Children’s Hospital. Also, this Agreement shall be effective upon signature by all parties and shall remain in effect for two (2) years from the effective date. All other terms and conditions from the original Agreement still apply. The Office of Epidemiology routinely requests and receives data from hospitals and healthcare providers in the course of disease investigations. (C-86-18-040-3-01)

AMENDMENT TO DATA USE AGREEMENT WITH CHILDREN’S HOSPITAL CORPORATION D/B/A BOSTON CHILDREN’S HOSPITAL Approve a retro-active Amendment No. 2 to the non-financial Data Use Agreement (Agreement) for Research between Children’s Hospital Corporation d/b/a Boston Children’s Hospital (BCH), a charitable corporation and Maricopa County by and through its Department of Public Health (MCDPH), Disease Control. This amendment will allow for continued access to anonymized data collected by Boston Children’s Hospital. This amendment shall remain in effect from December 29, 2020 through December 28, 2022. All other terms and conditions from the original Agreement and amendment 1 still apply.
