IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES Approve an Intergovernmental Agreement (IGA) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH), for the development and enhancement of the Behavioral Risk Factor Surveillance System (BRFSS). This IGA will provide reimbursement to ADHS for the provision of additional questionnaire data, in an amount not-to-exceed $7,100.00 for the budget period January 1, 2018 through December 31, 2018, with the option of renegotiating the Scope of Work and Pricing through-out the 5 year term of this agreement.
