AMENDMENT TO CONTRACT WITH SOUTHWEST HUMAN DEVELOPMENT FOR COMPREHENSIVE COMMUNITY HEALTH NURSING SERVICES Approve Amendment 4 to Contract between Southwest Human Development (SWHD) and Maricopa County through its Department of Public Health, Office of Family Health for comprehensive community health nursing services to eligible women and their families through the Nurse Family Partnership Program (NFP). The not-to-exceed (NTE) amount is $499,231 for the budget period of October 1, 2020 through September 30, 2021. The contract term is October 1, 2017 through September 30, 2022. This Contract is funded through an ADHS IGA grant, Nurse Family Partnership for Maternal, Infant and Early Childhood Home Visiting (MIECHV) (ADHS17-155910)) and does not affect the County general fund. This subcontractor is subject to the Maricopa County Procurement Code pursuant to Section MC1-102C, as they are a prior designated subcontractor. (C-86-18-048-3-04)

AMENDMENT TO CONTRACT WITH SOUTHWEST HUMAN DEVELOPMENT FOR COMPREHENSIVE COMMUNITY HEALTH NURSING SERVICES Approve Amendment 5 to Contract between Southwest Human Development (SWHD) and Maricopa County through its Department of Public Health, Office of Family Health, for comprehensive community health nursing services to eligible women and their families through the Nurse Family Partnership Program (NFP). The not-to-exceed (NTE) amount is $249,615.50 for the 6-month budget period of October 1, 2021 through March 31, 2022. This Contract is funded through an ADHS IGA grant, Nurse Family Partnership for Maternal, Infant and Early Childhood Home Visiting (MIECHV) (ADHS17-155910)) and does not affect the County general fund. This subcontractor is subject to the Maricopa County Procurement Code pursuant to Section MC1-102C, as they are a prior designated subcontractor. MCDPH and SWHD will continue to implement the Nurse Family Partnership (NFP) model in the South Phoenix Region to provide comprehensive community health nursing services to eligible women and their families. The program is called South Phoenix Nurse Family Partnership (SPNFP). MCDPH and SWHD are partnering in this endeavor as each organization has unique assets that will strengthen the implementation and success of this program. SWHD has been a provider of services for over 20 years in the South Phoenix area addressing the area’s multiple needs. SWHD provides Arizona Early Intervention Services and has been providing Healthy Families (HF) services in Maricopa County for 18 years. (C-86-18-048-3-05)
