36. NOTICE OF AWARD AND AGREEMENT WITH VITALYST HEALTH FOUNDATION Approve a Notice of Grant Award and agreement between Vitalyst Health Foundation (Vitalyst) and Maricopa County by and through its Department of Public Health (MCDPH) to provide funding for the Live Well Arizona mini-grant. Funding is in the not-to-exceed amount of $14,990 for the 6 month budget period beginning at execution of the agreement by County. MCDPH’s indirect rate for FY 2017-18 is 16.6%. The full indirect costs are estimated at $2,134.00, all of which is fully-recoverable. Departmental indirect rates are reestablished at the beginning of each fiscal year and the future indirect rates will be collected at the corresponding rates. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-18-066-3-00)

52. AMENDMENT TO NOTICE OF AWARD AND AGREEMENT WITH VITALYST HEALTH FOUNDATION Approve an amendment to the Grant Award and Agreement between Vitalyst Health Foundation (Vitalyst) and Maricopa County by and through its Department of Public Health (MCDPH) to extend the project/budget period from 6 months to 1 year. This funding provides for Sunnyslope community engagement, improving SMART Circulator Access and collaboration with community partners in building healthy community capacity. MCDPH has received grant awards from Vitalyst in the past though not on a reoccurring basis. The cash or in-kind match is not applicable and indirect costs are fully recoverable. Ongoing cash contributions are not required. The grant award is not a mandated function but provides a benefit to the citizens through collaborations with residents and partners in the Sunnyslope area. The grant award is a competitive opportunity. There are no costs that will need to be absorbed by the department’s operating budget. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law.
