45. AMENDMENT TO CONTRACT WITH SOUTHWEST HUMAN DEVELOPMENT FOR NURSE FAMILY PARTNERSHIP - HOME VISITATION Approve amendment 3 between Southwest Human Development (SWHD), Nurse Family Partnership (NPR), Home Visitation (HV) and Maricopa County by and through its Department of Public Health (MCDPH) to provide continued funding for the Nurse Family Partnership (NFP) program for the East Maricopa Region. The SWHD NFP program has adopted and implemented the NFP model in the East Maricopa Region and has been providing comprehensive community health nursing services to eligible women and their families. This Amendment 3 shall extend the term of the contract to June 30, 2023 and increase the total not-to exceed amount to $734,855 which is equal to $146,971 for each of the five budget periods covered by this agreement. The original term of this Agreement was July 1, 2018, through June 30, 2022 but was renewable for up to three (3) additional twelve (12) month extensions. This contract is funded through an Arizona Early Childhood Development and Health Board (AECDHB), First Things First (FTF), Home Visitation, Nurse Family Partnership, East Maricopa grant. The FTF grant award is recurring and has been awarded in previous years. This grant does not require an in-kind match; has a maximum of 10% indirect cost reimbursement as delegated in the Grant Agreement; and ongoing cash contributions are also not required. The grant award is not a mandated function but provides a benefit to the citizens by providing intensive family support to 50 high risk pregnant women. In addition, age-appropriate developmental and sensory screenings will be provided to 50 children of enrolled families throughout the first 2 years of their life. The grant award is competitive. There are no costs that will need to be absorbed by the department’s operating budget. (C-86-19-004-3-03)
