DATA USE AGREEMENT WITH CENTERS FOR DISEASE CONTROL, NATIONAL HEALTHCARE SAFETY NETWORK Approve this non-monetary Data Use Agreement between Maricopa County by and through its Department of Public Health (MCDPH), Epidemiology Department, and Centers for Disease Control (CDC), National Healthcare Safety Network (NHSN). This Agreement allows MCDPH access to National Healthcare Safety Network (NHSN) data reported by Maricopa County Healthcare Facilities (Maricopa County Acute Care Hospitals). MCDPH will collaborate with ADHS on outreach projects targeted to reduce the burden of Healthcare Associated Infections in Maricopa Healthcare Facilities. ADHS has entered into a similar agreement with CDC which allows ADHS to share data with MCDPH as well. This is a 5-year Agreement, effective upon full execution.
