39. LICENSE AGREEMENT WITH THE CITY OF AVONDALE Approve and authorize the Chairman to execute License Agreement P-50323 with the City of Avondale (the “Agreement”) for Maricopa County (“County”) Public Health Department (“PHS”) use of the Care1st Avondale Resource and Housing Center at 328 W. Western Avenue, Avondale, AZ 85323. The Agreement will allow PHS First Teeth First program work space to enhance the oral health status of children birth through age 5 and expectant mothers by providing basic oral health screenings, education and fluoride varnish. The vision is to reduce the prevalence of early childhood tooth decay and the associated risks for pain and infections that can lead to lifelong complications to health and well-being. The Agreement will commence upon execution by all parties and continue until terminated via 30-day written notice from either party to the other. There is no cost to the County or to presenting clientele. (C-86-19-042-L-00)
