PARTNERSHIP AGREEMENT WITH DIGNITY COMMUNITY CARE FOR IMMUNIZATIONS Approve a Partnership Agreement (PA) between Maricopa County by and through the Department of Public Health (MCDPH) and Dignity Community Care dba Chandler Regional Medical Center to provide vaccines to eligible children and adults at community based locations throughout Maricopa County. The term of this Agreement is May 1, 2019 through April 30, 2020 with automatic annual renewals. Either party may terminate this Agreement at any time. (C-86-19-044-3-00)
