DATA USE AGREEMENT WITH SOUTHWEST BEHAVIORAL & HEALTH SERVICES FOR THE COMMUNITY HEALTH DASHBOARD Approve this non-monetary Data Use Agreement between Maricopa County by and through its Department of Public Health (MCDPH), Epidemiology Department and Southwest Behavioral & Health Services for a dashboard that will enable community members to learn about the health of the citizens of Maricopa County. This Agreement details the criteria for sharing of data between parties to the agreement. This is a 5-year agreement, effective upon full execution. Either party may terminate with written notice.
