24. AMENDMENT TO CONTRACT WITH ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS (AACHC) FOR ASSESSMENT OF SUBSTANCE ABUSE INFRASTRUCTURES Approve Amendment No. 5 to RFP Contract #200142, between Maricopa County by and through its Department of Public Health and Arizona Alliance for Community Health Centers, to continue to provide assessment of Substance Abuse Infrastructures in Federally Qualified Community Health Centers. The agreement amount has been increased to add additional funds to stigma series and best practice series. The revised amount includes an increase of not-to-exceed amount of $100,000, bringing the amount from $801,000 to a total not to exceed amount of $901,000 for year 3 budget period beginning September 1, 2021 to August 31, 2022. The term of this contract is from September 1, 2019 thru August 31, 2022. It is mutually agreed the referenced contract is amended as follows: I. ATTACHMENT A, Scope of Work is revised and shall become effective on September 1, 2021. II. ATTACHMENT B, budget justification narrative is revised to reflect an increase in total project costs from $801,000 to $901,000. A ($100,000 increase) for the year 3 budget period beginning September 1, 2021 to August 31, 2022. III. All other terms and conditions of the original contract shall remain in full force and effect. AMENDMENT TO CONTRACT WITH ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS (AACHC) FOR ASSESSMENT OF SUBSTANCE ABUSE INFRASTRUCTURES Approve Amendment No. 6 to RFP Contract #200142, between Maricopa County by and through its Department of Public Health and Arizona Alliance for Community Health Centers, to continue to provide assessment of Substance Abuse Infrastructures in Federally Qualified Community Health Centers. The agreement has been extended through August 1, 2023. The amendment includes a revised scope of work and associated budget of $258,000 for the budget period of September 1, 2022 through August 31, 2023. The previous amendment had a total not to exceed amount of $901,000 bringing the new cumulative not-to-exceed amount to $1,159,000. This revised amount is for the continuation of Year 3 of the grant which is Year 4 of the grant in real-time. The term of the original contract is September 1, 2019 thru August 31, 2023. 68. AMENDMENT TO CONTRACT WITH ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS (AACHC) FOR ASSESSMENT OF SUBSTANCE ABUSE INFRASTRUCTURES Approve Amendment No. 7 to RFP Contract #200142, between Maricopa County by and through its Department of Public Health and Arizona Alliance for Community Health Centers, to continue to provide assessment of Substance Abuse Infrastructures in Federally Qualified Community Health Centers. The above-named contract is hereby amended as specified below: I. An additional $116,031 was added to the previously approved $258,000 for budget period of September 1, 2022, through August 31, 2023. This brings the total NTE for this budget period to $374,031. II. The Scope of Work has been modified to add additional work as shown on Page 2 of the amendment. III. A revised Budget Justification Narrative was added as shown on Page 3 of the amendment. All other terms and condition of the original contract and its amendment shall remain in full force and effect. The total not to exceed amount shall be $1,275,031 for the term of the entire contract. The term date of this contract is September 1, 2019, thru August 31, 2023. The revised amount is for the continuation of Year 4 of the grant. Arizona Alliance for Community Health Centers was the sole respondent to PH RFP 200142 Solicitation that closed on September 30, 2019. Due to their current infrastructure as well as their ability to fulfill the expectations of this contract, it was determined that a second solicitation was not in the best interest of the County.
