79. AMENDMENT TO CONTRACT WITH ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS (AACHC) FOR ASSESSMENT OF SUBSTANCE ABUSE INFRASTRUCTURES Approve amendment No. 1 to PH RFP Contract #200142, between Maricopa County, a political subdivision of the State of Arizona, through its Department of Public Health (MCDPH) and Arizona Alliance for Community Health Centers (“Contractor”), to continue to provide assessment of Substance Abuse Infrastructures in Federally Qualified Community Health Centers. The original term of this contract is for three (3) years, beginning December 1, 2019 and ending November 30, 2022. The revised contract term dates shall be changed to (September 1, 2020 – August 31, 2022). It is mutually agreed that the referenced contract is amended as follows A. Attachment A, Scope of Work. This updated scope of work shall become effective on September 1, 2020. B. Attachment B, Budget Justification. This updated year two (2) budget narrative is effective from September 1, 2020 to August 31, 2021. C. Attachment C, Year-two (2) Timeline. This updated timeline shall become effective on September 1, 2020. D. Contract term dates shall be changed to (September 1, 2020 – August 31, 2022). II. All other terms and conditions of the original contract shall remain in full force and effect. Arizona Alliance for Community Health Centers was the sole respondent to PH RFP 200142 Solicitation that closed on September 30, 2019. Due to their current infrastructure as well as their ability to fulfill the expectations of this contract, it was determined that a second solicitation was not in the best interest of the County. AMENDMENT TO CONTRACT WITH ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS (AACHC) FOR ASSESSMENT OF SUBSTANCE ABUSE INFRASTRUCTURES Approve a retroactive Amendment No. 2 to PH RFP Contract #200142, between Maricopa County through its Department of Public Health (MCDPH) and Arizona Alliance for Community Health Centers (“Contractor”), to continue to provide assessment of Substance Abuse Infrastructures in Federally Qualified Community Health Centers. The agreement amount has been increased to add additional funds to the consultant and other costs categories. The revised amount includes an increase of $168,000, bringing the amount from $150,000 to a total not to exceed amount of $318,000 for year two budget period beginning September 1, 2020 to August 31, 2021. The term of this contract is from September 1, 2020 – August 31, 2022. It is mutually agreed the referenced contract is amended as follows: I. ATTACHMENT B, budget justification narrative is revised to reflect an increase in total project costs from $150,000 to $318,000. This is a $168,000 increase for the year two budget period beginning September 1, 2020 to August 31, 2021. II. 68. AMENDMENT TO CONTRACT WITH ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS (AACHC) FOR ASSESSMENT OF SUBSTANCE ABUSE INFRASTRUCTURES Approve Amendment No. 7 to RFP Contract #200142, between Maricopa County by and through its Department of Public Health and Arizona Alliance for Community Health Centers, to continue to provide assessment of Substance Abuse Infrastructures in Federally Qualified Community Health Centers. The above-named contract is hereby amended as specified below: I. An additional $116,031 was added to the previously approved $258,000 for budget period of September 1, 2022, through August 31, 2023. This brings the total NTE for this budget period to $374,031. II. The Scope of Work has been modified to add additional work as shown on Page 2 of the amendment. III. A revised Budget Justification Narrative was added as shown on Page 3 of the amendment. All other terms and condition of the original contract and its amendment shall remain in full force and effect. The total not to exceed amount shall be $1,275,031 for the term of the entire contract. The term date of this contract is September 1, 2019, thru August 31, 2023. The revised amount is for the continuation of Year 4 of the grant.
