AMENDMENT TO IGA WITH ARIZONA BOARD OF PHARMACY Approve an Amendment No. 1 to Intergovernmental Agreement between Arizona Board of Pharmacy and Maricopa County by and through its Department of Public Health to provide continued funding for an epidemiologist position to support the collection and analysis of Prescription Drug Monitoring Program Data. The term of this Agreement is September 1, 2019 through August 31, 2022. I. The above named IGA contract is hereby amended as specified below: 3. Agreement Amount: change the agreement amount total from $150,000 to $89,632 (decrease by $60,368.00 for the budget period September 1, 2020 through August 31, 2021); II. ATTACHMENT A is added as Budget III. ATTACHMENT B is added as Budget Narrative IV. All other terms and conditions of the original contract and its amendments shall remain in full force and effect. AMENDMENT TO IGA WITH ARIZONA STATE BOARD OF PHARMACY Approve Amendment No. 2 to Intergovernmental Agreement (IGA) between Arizona State Board of Pharmacy and Maricopa County by and through its Department of Public Health. The term of this Agreement is from September 1, 2019 through August 31, 2022. This amendment includes the following modifications: I. The above named IGA is hereby amended as specified on page 14, Section IV, Compensation. Revise to read in its entirety: 1 . COMPENSATION A. The Contractor shall submit a quarterly invoice on the last business day of the month following the end of each annual contract quarter. For year two (2), invoices should be sent on or before the following dates: a. December 31, 2020 (for quarter ending November 30, 2020) b. March 31, 2021 (for quarter ending February 29, 2021) c. May 31, 2021 (for quarter ending May 31, 2021) d. September 30, 2021 (for quarter ending August 31, 2021) Each subsequent year will adhere to this same schedule. II. 73. AMENDMENT TO IGA ARIZONA BOARD OF PHARMACY Approve Amendment No. 5 to the Intergovernmental Agreement (IGA) between Arizona Board of Pharmacy and Maricopa County by and through its Department of Public Health. The Center of Disease Control and Prevention has extended the OD2A grant for one additional year, from September 1, 2022 through August 31, 2023. The term date for the grant was originally intended to end August 31, 2022 but the CDC has decided to extend Year 3 for additional time for subrecipients to meet its deliverables. This agreement supports the Arizona Board of Pharmacy’s Prescription Monitoring Program. This amendment adds $110,393 for the budget period September 1, 2022 thru August 31, 2023, bringing the not-to-exceed total from $494,025 to $544,050 for extension of the OD2A grant. This amendment also includes funding for the subrecipient team member to attend the Rx and Illicit Drug Summit conference and the National Association of State Controlled Substances Authorities (NASCSA) Conference totaling $7,100 for travel purposes. The compensation schedule has also been amended to reflect the extension of Year 3 of the OD2A grant.
