CONTRACT WITH MAYO CLINIC ARIZONA DBA MAYO CLINIC HOSPITAL FOR COMMUNITY HEALTH ASSESSMENT REPORT Approve a Contract for Services between Mayo Clinic Arizona dba Mayo Clinic Hospital and Maricopa County by and through its Department of Public Health (MCDPH) for participating in a Community Health Needs Assessment (CHNA). Mayo Clinic shall pay the County for the services described, in the amount of $13,000 per year, for three years, for a total not-to-exceed amount of $39,000. The term of this Contract is from July 1, 2020 through June 30, 2023 The Contract may be extended for one (1) additional three-year period, not to exceed a total extended term of six (6) years The Department of Public Health’s FY2019-20 indirect rate is 19.2%, indirect costs are estimated at $2,094 (per year) and are fully recoverable.
