69. AMENDMENT TO LEASE AGREEMENT WITH ROOSEVELT ELEMENTARY SCHOOL DISTRICT NO. 66 Approve and execute Amendment No. 1 to Lease Agreement L-7487 with Roosevelt Elementary School District No. 66 (“District”) for County use of premises located at 4615 S. 22nd Street, Phoenix. The purpose of this amendment is to extend the term of the agreement for two (2) years commencing on July 1, 2021 through June 30, 2023; adjust the monthly rent schedule: 2022 annual rate $4,200 and 2023 annual rate $4,668.00; and allow for administrative authority for County. The Agreement is subject to termination pursuant to the provisions of A.R.S. 38-511. This Agreement may be terminated by Tenant at the end of any fiscal year due to non-appropriation of funds without any penalty or liability to Tenant. County and state fiscal years end June 30, Federal fiscal year ends September 30. Either party may terminate this Agreement with a 90-day written notice. Use of the premises is for the operation of a WIC facility, including associated strategic partners. Operational hours are from 7:30 am to 4:30 pm Monday through Friday and by special arrangement of with District. This site is located in Supervisorial District 5. (C-86-20-057-1-01)
