20. AMENDMENT TO IGA WITH ARIZONA BOARD OF REGENTS FOR AND ON BEHALF OF ARIZONA STATE UNIVERSITY FOR COVID RESPONSE ACTIVITIES Approve amendment 2 to Intergovernmental Agreement (IGA) between Arizona Board of Regents for and on behalf of Arizona State University (ASU) and Maricopa County by and through its Department of Public Health (MCDPH). With these scope of work enhancements ASU will continue to provide COVID-19 response services to include Case Investigations and other assistance, as detailed within the amendment. This amendment is retroactive beginning on January 1, 2022. The project period is extended through June 30, 2022 for a total project period of July 1, 2020 through June 30, 2022. The total not to exceed amount for all services under this IGA is now $2,428,230. Cost for Exhibit D Case Investigation, is increased by $486,397 and Exhibit G Special Project, is increased by $46,137 for the period of January 1, 2022 thru June 30, 2022. Funding for this IGA is provided by the ‘COVID-19 Disease Mitigation and Community Outreach”, IGA CTR055638, which is to end on May 1, 2023. AMENDMENT TO IGA WITH ARIZONA BOARD OF REGENTS FOR AND ON BEHALF OF ARIZONA STATE UNIVERSITY FOR COVID RESPONSE ACTIVITIES Approve Amendment 3 to Intergovernmental Agreement (IGA) between Arizona Board of Regents for and on behalf of Arizona State University (ASU) and Maricopa County by and through its Department of Public Health (MCDPH). With these Scope enhancements and surge capacity support, ASU will continue to provide COVID-19 response services to include Case Investigations, Modeling & Analysis, Serosurvey, and Qualtrics questionnaire, as detailed within the amendment. This amendment is effective upon Board approval. The total budget for Exhibit G is being increased by $53,448 for the period of January 1, 2022 – June 30, 2022. This brings the total cumulative not-to-exceed amount to $380,279 for the period of January 1, 2020 through June 30, 2022. 84. AMENDMENT TO IGA WITH ARIZONA BOARD OF REGENTS FOR AND ON BEHALF OF ARIZONA STATE UNIVERSITY FOR COVID RESPONSE ACTIVITIES Approve Amendment 4 to Intergovernmental Agreement (IGA) between Arizona Board of Regents for and on behalf of Arizona State University (ASU) and Maricopa County by and through its Department of Public Health (MCDPH). The amendment extends the agreement end date and Period of Performance from June 30, 2022 mentioned in Section 3.1 to December 30, 2022. The amendment also modifies the Scope of Work in Exhibit D in the following manner: In the Case Investigation Section- reduction in the number of cases investigated by ASU from 125 to 25 daily. The price sheet includes an additional column in Exhibit D by $163,280 to cover the period of performance from July 1, 2022 through December 30, 2022. Also, the Scope of Work in Exhibit G is modified in the following manner: Additional column to increase funding for the Special Projects scope of work. Exhibit G functions to support Community Support and Outreach efforts, including a questionnaire for a data collection. The increase in funding is in the amount of $500,000 for the period of performance July 1, 2022 through December 30, 2022. This amendment is effective upon Board approval. This brings the not-to-exceed amount to for both Exhibits D and G to $663,280. Funding for this IGA is provided by the ‘COVID-19 Disease Mitigation and Community Outreach”, IGA CTR055638, which is to end on May 1, 2023.
