AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HEALTHY PEOPLE HEALTHY COMMUNITIES Approve Amendment 2 to fixed-price Intergovernmental Agreement (IGA) No. CTR055429 between Arizona Department of Health Services (ADHS), Healthy People Healthy Communities and Maricopa County by and through its Department of Public Health (MCDPH). The purpose of this Agreement is to provide continued funding for evidence-based strategies related to specific program areas set by ADHS. The program areas continuing in this amendment include the Tobacco Program, Health in Arizona Policy Initiative (HAPI), Teen Pregnancy Prevention Program (TPP), Youth Mental Health First Aid Initiative, and the Suicide Mortality Review Program. This amendment revises and replaces several areas in the Scope of Work. Major changes include the movement of the Public Health Improvement Program and the Healthy Arizona Worksite Program (HAWP) out of this IGA. However, specific Worksite Wellness activities will be moved to the existing HAPI scope of work/action plan. Exhibits A through E are revised and replaced to reflect these changes. The Price Sheet has been revised and replaced and reflects a total not-to-exceed amount of $3,062,829 for the budget period beginning July 1, 2022, through June 30, 2023. The term of the original Agreement is July 1, 2020, through June 30, 2025. The individual price sheet budget amounts are: Action Plan: $339,541 Tobacco: $1,953,616 HAPI: $379,804 TPP: $183,868 TPP - Youth Mental Health First Aid Initiative: $6,000 Suicide Mortality Review Program: $200,000 MCDPH’s indirect rate for FY23 is 17.24%.
