CONTRACT WITH VALLEYWISE HEALTH MEDICAL CENTER FOR COMMUNITY HEALTH NEEDS ASSESSMENT REPORT Approve a Contract for Service between Valleywise Health Medical Center and Maricopa County by and through its Department of Public Health for participating in a Community Health Needs Assessment (CHNA). Valleywise Health Medical Center shall pay the County for the services described, in the amount of $10,000 per year, for three years, for a total not-to-exceed amount of $30,000. The term of this Contract is from August 1, 2020 through August 1, 2023. The Contract may be extended for one (1) additional three-year period, not to exceed a total extended term of six (6) years The Department of Public Health’s FY20 indirect rate is 18.8%, indirect costs are estimated at $1,582 (per year) and are fully recoverable.
