45. CONTRACT WITH NEIGHBORHOOD OUTREACH ACCESS TO HEALTH FOR COMMUNITY HEALTH NEEDS ASSESSMENT REPORT Approve a retroactive Contract for Service between Neighborhood Outreach Access to Health (NOAH) and Maricopa County by and through its Department of Public Health (MCDPH) to participate in a Community Health Needs Assessment (CHNA). The purpose of the CHNA is to assess the public health needs of Maricopa County residents. Neighborhood Outreach Access to Health is a Federally Qualified Health Center (FQHC) and shall pay the County for services described, in yearly installments of $7,000 for year one, $10,000 for year two and $10,000 for year three, over the term of this contract for a total not-to-exceed amount of $27,000. The term of this Contract is from July 1, 2020 through June 30, 2023. The Contract may be extended for one (1) additional three-year period, not to exceed a total extended term of six (6) years The Department of Public Health’s FY20 indirect rate is 18.8%, indirect costs are estimated at $1,108 for year one (1), $1,582 for year two (2), and $1,582 for year three (3), and are fully recoverable. 56. BUSINESS ASSOCIATE AGREEMENT WITH NEIGHBORHOOD OUTREACH ACCESS TO HEALTH Approve a retroactive Business Associate Agreement (BAA) with Neighborhood Outreach Access to Health (NOAH) through its affiliates and Maricopa County by and through its Department of Public Health (MCDPH). The purpose of the BAA is to establish parameters for a data sharing agreement between MCDPH and NOAH. The metrics and methods of sharing HIPAA-Protected Health Information will enhance information sharing for the Community Health Needs Assessment (CHNA) survey, which is conducted every three years. The BAA will utilize the Synapse contract and will result in more comprehensive data for both parties. NOAH is a community partner within the Community Transformation division of Public Health. This is a non-financial agreement between both parties. The term of this Agreement shall begin retroactively on July 1, 2020 and will continue in perpetuity, until terminated by either party.
