CONTRACT WITH DIGNITY HEALTHCARE FOR COMMUNITY HEALTH NEEDS ASSESSMENT REPORT Approve a retroactive Contract for Service between Dignity Health and Maricopa County by and through its Department of Public Health (MCDPH) to participate in a Community Health Needs Assessment (CHNA). The purpose of the CHNA is to assess the public health needs of Maricopa County residents. Dignity Health is a Federally Qualified Health Center (FQHC), and shall pay the County for services described, in equal yearly installments of $89,000 over the term of the contract for a total not-to-exceed amount of $267,000. The term of this Contract is from August 1, 2020 through June 30, 2023. The Contract may be extended for one (1) additional three-year period, not to exceed a total extended term of six (6) years. The participating hospital facility locations under Dignity Health are: Hospitals • St. Joseph’s Hospital and Medical Center • St. Josephs’ Westgate • Chandler Regional Medical Center • Mercy Gilbert Medical Center • Arizona General – Laveen • Arizona General – Mesa Joint Ventures • Arizona Orthopedic Specialty Hospital (AOSH) • Arizona Spine and Joint • Kindred: Dignity Health East Valley Rehabilitation Hospital • OASIS $ The Department of Public Health’s FY20 indirect rate is 18.8%, indirect costs are estimated at $42,253 and are fully recoverable.
