55. PARTNERSHIP AGREEMENT BETWEEN MARICOPA COUNTY AND PARTNER SCHOOL DISTRICTS FOR IMMUNIZATION PROGRAM Approve an agreement to be executed between Maricopa County by and through the Department of Public Health (MCDPH) and partner School Districts through-out Maricopa County. This agreement is in final form. The term of this Agreement becomes effective on the date both parties have signed and will aut omatically renew annually when agreed to by both parties. Either party may terminate this Agreement at any time. Authorize MCDPH Director or Deputy Director to execute these agreements. Should the document be altered in any manner after Board approval, it will be placed on the next available meeting date for further review and approval. To assure that children receive the vaccines needed to attend school, Maricopa County Department of Public Health will continue to enter into agreements with school distri cts to provide vaccines to children who are eligible for Vaccine for Children vaccines and to children who have private insurance with a plan that is contracted with Maricopa County Department of Public Health. 71. AMENDMENT TO SCHOOL DISTRICT PARTNERSHIP AGREEMENT FOR THE IMMUNIZATION PROGRAM WITH CARTWRIGHT ELEMENTARY SCHOOL DISTRICT Approve Amendment 2 to the Partnership Agreement (PA) between Maricopa County, through its Department of Public Health (MCDPH), and partner school district, Cartwright Elementary School District, for the continued provision and administration of all age appropriate vaccines for children, Flu and COVID-19 vaccines for staff and accompanying parents/guardians based on qualifying age regardless of insurance status and, upon availability from community partners, the Vaccines for Adults (VFA) program for staff and accompanying parents/guardians who are uninsured or underinsured for vaccines. This PA is non-financial. The Agreement shall automatically renew annually with written confirmation by both parties and either party may terminate at any time. In order to address needs of Cartwright Elementary School District, MCDPH will execute this Amendment 2 to formally document the Agreement term is hereby extended through June 30, 2023. All other terms and conditions of the original contract shall remain in full force and effect. (C-86-21-063-X-02)
