76. AMENDMENT TO IGA WITH CITY OF PHOENIX FOR MASS IMMUNIZATION SERVICES AND ADMINISTRATION Rescind Amendments 2 & 3 approved on November 17, 2021 (C-86-21-082-X-02) and January 5, 2022 (C-86-21-082-X-03) and replace with a revised Amendment 2 to the Intergovernmental Agreement (IGA) between Maricopa County, through its Department of Public Health (MCDPH) and City of Phoenix for Mass Immunization services. This Amendment will continue to allow the City of Phoenix to provide and administer COVID-19 and seasonal flu vaccines (as well as other types of vaccines, as needed) to those 6 months of age and older. Amendment 2 shall extend the Expiration date to March 31, 2023. In addition, it revises the Compensation section to indicate that reimbursement rates shall be subject to change and current rates will be communicated via a link on the MCDPH COVID-19 Vaccination Vendor Resources website. This agreement is funded through the Arizona Department of Health Services Immunization Equity funding (ADHS18-177694) effective through the term ending June 30, 2024, approved as C-86-18-018-3-13 on 8/18/2021. Should these funds cease, and alternate funding not be available, IGA Section 10 allows for termination of the contract. (C-86-21-082-X-04)
