87. LEASE AGREEMENT WITH NORTH PHOENIX BAPTIST CHURCH FOR A WOMEN, INFANTS AND CHILDREN (WIC) CLINIC Approve and execute Lease Agreement L-7514 between North Phoenix Baptist Church (Lessor) and Maricopa County (Lessee) for Maricopa County Public Health Healthy Start Program’s use of 1,575 rentable square feet (RSF) of office and clinic space at 5757 N. Central Avenue, Phoenix, Arizona; and authorize the Assistant County Manager and/or the County Real Estate Director to administer the Agreement. The commencement date is January 1, 2022 and will expire on December 30, 2024. Upon mutual agreement, the lease may be extended for two (2) additional terms of two (2) years each. The lease rate is $131.25 per month ($1,575.00 annually) with an additional $320.00 per month for operation expenses, and there will be no rental tax charged. This site is located in Supervisory District 3 (C-86-22-077-X-00)
