26. CONTRACT WITH BANNER POISON AND DRUG INFORMATION CENTER FOR THE PURCHASE OF PUBLIC HEALTH DISEASE/EMERGENCY REPORTING LINE SERVICES Approve a Public Health Contract #PH RFP 22009, between Maricopa County (County), a political subdivision of the State of Arizona, through its Department of Public Health (MCDPH) and Banner Poison and Drug Information Center, (“Contractor”) to provide Public Health Disease/Emergency Reporting Line services. This contract is being entered into pursuant to MC1-329 of the Maricopa County Procurement Code. The initial term of the contract is for three (3) years, beginning retroactively from the 1st day of January 2022 and ending the 31st day of December 2024. The total anticipated cost of the service is $459,422, which includes $100,000 annual base rate for years 1 through 3 and an additional $159,422 in contingency rates for year 1 for nurses and medical doctors which are set at $62.00 per hour and $150.00 per hour respectively. The County may, at its option and with the agreement of the Contractor, renew the term of this Contract for additional terms up to a maximum of two (2) additional years, (or at the County’s sole discretion, extend the contract on a month-to-month bases for a maximum of six (6) months after expiration).
