47. CONTRACTS FOR BUILDING HEALTHY AND RESILIENT COMMUNITIES IN THE SOUTHWEST REGION OF MARICOPA COUNTY Pursuant to MC1-329 of the Maricopa County Procurement Code, authorize the Chairman of the Board of Supervisors to execute one or more Contracts to be developed under #PH RFP 220199, Building Healthy and Resilient Communities - Southwest Region. The funding will be used to help develop and work through strategies and interventions that consider systemic barriers and potentially discriminatory practices. Funding must support Policy, System and Environmental changes (PSE) that can impact the disparity gap for vulnerable and underserved populations. Funds are not intended for direct services or direct financial distribution to the public. Funds are intended to assist populations instead of individuals. This solicitation (PH RFP 220199) is intended for residents in the Southwest Region which identified mental health issues, obesity, and substance abuse as priority areas from the completed Community Health Needs Assessment in July 2020 and successful respondents are required to address at least one of these priorities. The selected Contractors will be determined qualified to provide services by the Public Health Department’s Evaluation Committee. The resulting Contracts are granted a term of fourteen (14) months, beginning April 1, 2022 through May 31, 2023, with an option to renew for one-year terms up to a maximum of four (4) additional years (or at the County’s sole discretion, extend the contract on a month-to-month bases for a maximum of six (6) months after expiration) based on available funding and successful Contractor performance. The total amount to be spent on all awarded contracts under this solicitation is expected to be $2,500,000 over the initial term (first project period) with no Contractor receiving more than $500,000. This agreement will be funded by the COVID-19 Health Improvement Plan- Disparities Grant. Should the funding source cease and alternate funding not be available, Section 4.0 of the Contract, Availability of Funds, will apply allowing for a change or termination of the contract.
