MOU WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR SHARING OF PUBLIC HEALTH DATA Approve a Memorandum of Understanding (MOU) No. HU156031 between Arizona Department of Health Services (ADHS), and Maricopa County by and through its Department of Public Health (MCDPH). This MOU sets forth and defines the terms under which ADHS and MCDPH agree to provide one another specific data as defined in section 2.2.1. The data to be provided include all those listed in the table in Exhibit A and as further defined and limited within any other exhibit attached to this agreement. The terms of the Memorandum of Understanding (MOU) will be for five (5) years commencing upon signature of both parties and remains in effect unless otherwise terminated by either party per Section 6. MOU WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR SHARING OF PUBLIC HEALTH DATA Approve Amendment 1 of the non-financial Memorandum of Understanding (MOU) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH). This MOU sets forth and defines the terms under which ADHS and MCDPH agree to provide one another specific data as defined in Section 2.2. The data to be provided include all those listed in the table in Exhibit A and as further defined and limited within any other exhibit attached to this agreement. The original term of the Agreement began February 15, 2022 and will continue through February 14, 2027. The above-named contract is hereby amended as specified below: 1. Pursuant to the Uniform Terms and Conditions, Provision Eleven (11), Amendment or Modifications, the Agreement is hereby revised with the following: 1.1 Section 2.2.4., Data Transfer Methods, of this Agreement is hereby revised and replaced; 1.2 Section 20., Notices, Correspondence, and Reports, of this Agreement is hereby revised and replaced; 1.3 Exhibit A is hereby revised and replaced; 1.4 Exhibit B is hereby revised and replaced; 1.5 Exhibit C is hereby revised and replaced; 1.6 Exhibit D is hereby revised and replaced; 1.7 Exhibit E is hereby revised and replaced; 1.8 Exhibit F is hereby revised and replaced; 1.9 Exhibit G is hereby revised and replaced; 1.10 Exhibit K is hereby revised and replaced; 1.11 Exhibit L is hereby revised and replaced; 1.12 Exhibit M is hereby revised and replaced; 1.13 Exhibit N is hereby revised and replaced; 1.14 Exhibit O is added; and 1.15 Exhibit P is added. Either Party may terminate this MOU, in whole or in part at any time before the Term expiration by giving thirty (30) days written notice to the other party. AMENDMENT TO MOU WITH ARIZONA DEPARTMENT OF HEALTH SERVICES Approve Amendment No. 2 of the non-financial Memorandum of Understanding (MOU) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH). The original term of the Agreement began February 15, 2022 and will continue through February 14, 2027. Effective upon signature by both parties it is mutually agreed that the Memorandum of Understanding referenced in this Amendment Two (2) is amended as follows: 1. Pursuant to the Terms and Conditions, Provision Eleven (11) Amendment or Modifications, the Agreement is hereby revised with the following: 1.1 Exhibit A is revised and replaced. 1.2 Exhibit Q is added. Either Party may terminate this MOU, in whole or in part at any time before the Term expiration by giving thirty (30) days written notice to the other party.
