78. CONTRACTS FOR COMMUNITY ADVISORY BOARD CONSULTANT FOR HEALTH DISPARITIES WITHIN MARICOPA COUNTY Pursuant to MC1-329 of the Maricopa County Procurement Code, authorize the Chairman of the Board of Supervisors to execute one or more cost-reimbursement, service Contracts to be developed under #PH RFP 220216, Community Advisory Board (CAB) Consultant – Health Disparities for Maricopa County. The selected Contractors will serve as Consultants who will support the five (5) Regional Community Advisory Boards (CAB) that were established by MCDPH to develop, maintain, and strengthen strong partnerships with community leaders and stakeholders to enhance public health support. In addition to supporting the activities of their designated CAB, the Consultant shall also assist in the creation and development of the Regional Health Improvement Plans (RHIP)/Community Action Plans (CAP). The resulting Contracts are granted an initial term of fourteen (14) months, beginning April 1, 2022 through May 31, 2023 with the option to extend for additional terms up to a maximum of four (4) additional years, or at the County’s sole discretion, extend the contract on a month-to-month basis for a maximum of six (6) months after expiration, based on available funding and successful Contractor performance. The total amount to be spent on all awarded contracts under this solicitation is expected to be $500,000 over the initial term (first project period) which is to be distributed among all 5 regions of Maricopa County. Should the funding source cease and alternate funding not be available, Section 4.0 of the Contract, Availability of Funds, will apply allowing for a change or termination of the contract.
