PREMISE USE LICENSE AGREEMENT WITH THE CITY OF SURPRISE FOR THE MARICOPA COUNTY DEPARTMENT OF PUBLIC HEATLH FIRST TEETH FIRST PROGRAM Approve and authorize a Premise Use License Agreement between the City of Surprise and Maricopa County, by and through its Department of Public Health ( MCDPH) for use of the Surprise Resource Center located at 12425 W Bell Road, Bldg. A, Suite #124, Surprise, Arizona and the Surprise Senior Center located at 15832 N Hollyhock St, Surprise, Arizona. This agreement will allow MCDPH’s Office of Oral Health, First Teeth First program workspace to enhance the oral health status of children birth through age 5 and expectant mothers by providing basic oral health screenings, education, and fluoride varnish. This non-financial agreement will commence upon execution by all parties and continue until terminated via 30-day written notice from either party to the other. There is no financial impact to the County. (C-86-22-130-X-00)
