86. AGREEMENT FOR ACCESS TO PROTECTED HEALTH INFORMATION WITH MARICOPA COUNTY SPECIAL HEALTH CARE DISTRICT DBA VALLEYWISE HEALTH Approve Data Access Agreement between Maricopa County Special Health Care District dba Valleywise Health and Maricopa County by and through its Department of Public Health (MCDPH), Office of Epidemiology. This is a non-financial agreement between the parties. The term of this Agreement shall become effective May 18, 2022 and remain in effect through May 17, 2025. This non-financial Agreement serves to allow MCDPH, Office of Epidemiology access to Valleywise Health electronic health records for the purpose of treatment, payment, and certain health care operations to the extent permitted without authorization by the Administrative Simplification subtitle of the Health Insurance Portability and Accountability Act of 1996 (collectively, “HIPAA”) ”), and further subject to the American Recovery and Reinvestment Act of 2009 (“ARRA”), including its provisions Health Information Technology for Economic and Clinical Health Act ( “HITECH Act,”) and rules and regulations promulgated thereunder, as may be amended from time to time.
