AMENDMENT TO RFP CONTRACT 200002 WITH COLLABORATIVE RESEARCH FOR RYAN WHITE PART A PROGRAM Approve Amendment 2 to RFP Contract 200002 with Collaborative Research for Ryan White Part A Program consulting services. This Amendment shall extend the Expiration date thru November 30, 2025. In addition, this Amendment shall increase the contract by an additional $1,400,000 for a not-to-exceed amount of $2,800,000 over the life of the contract. The consulting services will continue to inform service delivery design for more than 12,000 people living with HIV in Maricopa County. It will also help to identify opportunities for efficiencies and improved health outcomes as we work to reduce new HIV infections in Maricopa County by 75%. The Base Contract was approved under Procurement Services and was entered into November 20, 2019. (C-86-22-168-X-00)
