49. RFP CONTRACTS FOR COMMUNITY TESTING SERVICES FOR COVID-19 Authorize the Chairman of the Board of Supervisors to execute, in a form approved by the County Attorney’s Office, Contracts to be developed under a re-solicitation of the Office of Procurement Services’ 210111-RFP. This re-solicitation (PH RFP 210111) is for ‘Community Testing Services for COVID-19’ and is related to the COVID-19 response. Chosen Contractors will be determined qualified to provide services by the Public Health Department’s Selection Committee. The Contracts are to be for a term of one (1) year and 10 months beginning November 3, 2021 with 2 one-year options for renewal. This is to coincide with the existing 16 Contractors that have already been issued contracts under the previous solicitation done by the Office of Procurement Services in October 2020. Authorize any amendments to all contracts developed and executed under Solicitation PH RFP 210111 to be signed by the Director, or the Director’s authorized representative, of the Maricopa County’s Office of Procurement Services. These agreements will be funded by the ELC2 grant, a COVID-19 related award, approved 7/14/2021 as C-86-22-194-X-00. Should that funding source cease, and alternate funding not be available, Section 4.0 of the Contracts, Availability of Funds will apply, allowing for a change or termination of the contract. 56. AMENDMENT TO COMPETITION IMPRACTICABLE CONTRACT WITH QUALTRICS, LLC FOR CONTRACT TRACING Pursuant to Procurement Authority Code MC1-352B, approve an amendment to a Competition Impracticable (CI) procurement contract with Qualtrics, LLC, in the amount of not-to-exceed $181,500 ($31,500 + $150,000) for Standard Messaging Service credits related to symptom tracking for contract tracing. The initial CI was approved by the Board of Supervisors 7/14/2021 as C-86-22-194-X-00 (item #83) and is currently scheduled to end August 5, 2022. The purpose of these funds is to provide support for contract tracing during the COVID-19 pandemic. This amendment will be funded through the ADHS ELC2 IGA COVID-19 Disease Mitigation and Community grant, CTR055638. 51. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR COVID-19 DISEASE MITIGATION AND COMMUNITY OUTREACH Approve an Amendment to Intergovernmental Agreement (IGA) Contract No. CTR055638 between the Arizona Department of Health Services (ADHS) and Maricopa Count by and through its Department of Public Health (MCDPH) to provide continued funding for the Covid-19 Disease Mitigation and Community Outreach efforts. This Amendment serves to correct the project start date as beginning January 15, 2021. The previous IGA incorrectly listed it as beginning March 1, 2021. All other provisions of the agreement remain unchanged. This was a new grant award and is not expected to reoccur. The grant does not require an in-kind match and ongoing cash contributions are not required. This grant award was not competitive. Any costs not covered by the grant will be absorbed by the departments indirect cost pool. The Department of Public Health indirect rate for FY22 is 17.97%. Funding for this IGA is provided by the ‘COVID-19 Disease Mitigation and Community Outreach”, IGA CTR055638, which is to end on May 1, 2023. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR COVID-19 DISEASE MITIGATION AND COMMUNITY OUTREACH Approve Amendment 3 to Intergovernmental Agreement (IGA) Contract No. CTR055638 between the Arizona Department of Health Services (ADHS) and Maricopa Count by and through its Department of Public Health (MCDPH). This is a no-cost-extension amendment to provide continued funding for the Covid-19 Disease Mitigation and Community Outreach efforts. The amendment serves to extend the Contract from May 1, 2023, to July 31, 2024. Moreover, the amendment has additionally reallocated the budget at our request. All other provisions of the agreement remain unchanged. Revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. Funding for this Agreement is provided by a Grant from ADHS and will not affect the County’s general fund. (C-86-22-194-X-03)

AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR ELC2 DISEASE MITIGATION AND COMMUNITY OUTREACH Approve Amendment 4 to Intergovernmental Agreement (IGA) Contract No. CTR055638 between the Arizona Department of Health Services (ADHS) and Maricopa Count by and through its Department of Public Health (MCDPH) for ELC2 Disease Mitigation and Community Outreach efforts. The above-named contracts are hereby amended as specified below: 1. Pursuant to the Terms and Conditions, Provision Four (4) Contract Administration and Operation, Section 4.2. Contract Renewal: the IGA is hereby extended through July 31, 2026, Year 5 of the contract. 2. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR DISEASE MITIGATION AND COMMUNITY OUTREACH Approve Amendment No. 5 to Intergovernmental Agreement (IGA) No. CTR055638 between the Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) for ELC2 Disease Mitigation and Community Outreach efforts. It is mutually agreed that the IGA referenced is amended as follows: 1. Pursuant to Terms and Conditions, Provision Six (6) Contract Changes, subsection 6.1 Amendments, the Contract is hereby revised with the following: 1.1 The Terms and Conditions are revised and replaced due to the Contract being extended past the maximum five (5) year term in Amendment Four (4). 1.2 The price sheet is revised and replaced to reflect updated funding allocations, including transfers between budget categories exceeding the ten percent (10%) threshold and funding of previously unfunded line items. All other terms and conditions of the original contract shall remain in full force and effect. (C-86-22-194-X-05)
