87. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES Approve Amendment number 1 for Intergovernmental Agreement (IGA) for contract number CTR056454 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) to provide continued funding for the Congenital Syphilis Intervention Project. The revised budget period of this contract has been extended to August 1, 2022 through August 1, 2023. Effective upon signature by both parties, it is mutually agreed that the IGA referenced in this amendment is amended as follows: 1. Pursuant to the Terms and Conditions, Provision Six (6), Contract Changes, 6.1., Amendments, Purchase orders and Change orders, the termination date is extended to August 1, 2023. The purpose of this project is to expand access to services to promote healthy pregnancies and prevent congenital syphilis. The term of the contract is 5 years, ending August 1, 2026. The award was non-competitive and there is no cash or in-kind match required. This is not a mandated service but provides a benefit to the community through the promotion of healthy pregnancies and the prevention of congenital syphilis. If discontinued, ongoing cash contributions should not be required. The Department of Public Health indirect rate for FY23 is 17.24%. Full indirect costs were estimated at $44,114.64, all of which were recoverable. Departmental indirect rates were reestablished at the beginning of each fiscal year and the future indirect rates will be collected at the corresponding rates. All program costs are allocated to the grant so there will not be additional burden on the department’s operating budget. The CFDA number is 93.323. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore, expenditure of the funds is not prohibited by the budget law. Approval of this action does not alter the budget constraining the expenditure of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. (C-86-22-231-X-01)

51. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES Approve Amendment number two (2) for Intergovernmental Agreement (IGA) CTR056454 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MSDPH) to provide continued funding for the Congenital Syphilis Intervention Project. The term of this contract has been extended to August 01, 2024, (the third year of the Contract) with a not-to-exceed amount of $300,000 for budget period August 01, 2023 through July 31, 2024. The term of the contract is 5 years, ending August 01, 2026. Effective upon signature by both parties, it is mutually agreed that the Intergovernmental Agreement referenced I this Amendment Two (2) is hereby amended as follows: 1. Pursuant to the Terms and Conditions, Provision Four (4), Contract Administration and Operation, 4.2 Contract Renewal, the Contract is extended through August 01, 2024, the (3rd) year of the Contract. 2. Pursuant to Terms and Conditions, Provision Six (6), Contract Changes, 6.1 Amendments, Purchase Orders and Change Orders, the Exhibit A-CFR200.332 Form is revised and replaced. The purpose of this project is to expand access to services to promote healthy pregnancies and prevent congenital syphilis. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES Approve retroactive Amendment number three (3) for Intergovernmental Agreement (IGA) CTR056454 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH) for the Congenital Syphilis Intervention Project. Effective January 1, 2024, it is mutually agreed that the Intergovernmental Agreement referenced in this Amendment Three (3) is hereby amended as follows: 1. Pursuant to the Terms and Conditions, Provision Six (6), Contract Changes, Section 6.1. Amendments, Purchase Orders and Change Orders, the Contract is amended as follows: 1.1. The Price Sheet is revised and replaced. This amendment shall increase the not to exceed (NTE) amount of the contract by $10,000; to a total of $310,000. 1.2.
