90. AMENDMENTS TO RFP CONTRACT 13054 WITH CHICANOS POR LA CAUSA AND AREA AGENCY ON AGING FOR RYAN WHITE PART A PROGRAM Approve Amendments to Contract 13054-RFP with Chicanos Por La Causa and Area Agency on Aging for Ryan White Part A Program for Medical and Non-Medical Case Management Services. This Amendment adds an additional deliverable to the scopes of work for the purchase of an Electronic Health Record system to be used by the Contractors. In addition, MCDPH requests the total maximum threshold for all contracts under this RFP to be increased by $3,000,000 for a new NTE amount of $8,910,175 to be used for the above stated purchases as well as existing services already being provided under the terms of the contracts. The case management services will continue to inform service delivery design for more than 12,000 people living with HIV in Maricopa County. It will also help to identify opportunities for efficiencies and improved health outcomes as we work to reduce new HIV infections in Maricopa County by 75%. The Base Contracts were approved under the Office of Procurement Services and were entered into on February 26, 2014. Currently they are scheduled to expire on 02/28/2024. (C-86-23-001-X-00)
