CONTRACT WITH CREIGHTON COMMUNITY FOUNDATION FOR THE PURCHASE OF SERVICES RELATED TO BUILDING HEALTHY AND RESILIENT COMMUNITIES – CENTRAL REGION INITIATIVE FOR MARICOPA COUNTY Approve an RFP contract by and between Maricopa County (“County”), a political subdivision of the State of Arizona, and Creighton Community Foundation, an Arizona Foundation (“Contractor”) for the purchase of services related to Building Healthy and Resilient Communities – Central Region Initiative. This Contractor was a successful respondent to #PH RFP 220200, Building Healthy and Resilient Communities – Central Region Initiative. The decision was later made to change the funding source of this particular proposal to be funded by the American Rescue Plan Act ( ARPA) so the community-based organization may execute its plan to impact change in policy and systems within Maricopa County. The total not-to-exceed amount is $499,981.27. The term of this agreement is 12 months, beginning on July 1, 2022 and ending May 31, 2023, with an option to renew for one-year terms up to a maximum of four (4) additional years (or at the County's sole discretion, extend the contract on a month-to-month bases for a maximum of six (6) months after expiration) based on available funding and successful Contractor performance. This agreement will be funded by the American Rescue Plan Act. Should the funding source cease and alternate funding not be available, Section 4.0 of the Contract, Availability of Funds, will apply allowing for a change or termination of the contract.
