PARTNERSHIP AGREEMENT WITH PEORIA UNIFIED SCHOOL DISTRICT FOR IMMUNIZATION PROGRAM Approve the Partnership Agreement (PA) between Maricopa County, by and through the Department of Public Health (MCDPH) and partner School District, Peoria Unified School District (PUSD). This Agreement will become effective at Board Approval for both part ies for the period of August 1, 2022, through May 31, 2023. This PA shall automatically renew annually with written confirmation by both parties and either party may terminate at any time. This PA is non-financial. The purpose of this Agreement is to dev elop a framework of cooperation between MCDPH and PUSD to provide childhood and the seasonal influenza vaccines to students who are uninsured, insured and the seasonal influenza vaccine for adult who are uninsured and insured.
