96. PH RFP 230080 CONTRACT FOR PROJECT ECHO COVID-19 RECOVERY EDUCATION AND CARE MANAGEMENT PROGRAM Pursuant to MC1-329 of the Maricopa County Procurement Code, authorize the Chairman of the Board of Supervisors to execute one or more Public Health Request for Proposal (PH RFP) contracts, in a form approved by the County Attorney’s Office, to one or more successful bidders in response to PH RFP 230080, Project ECHO COVID-19 Recovery Education and Care Management Program. Project ECHO (Extension for Community Healthcare Outcomes) is a collaborative model of medical education and care management that helps clinicians provide expert-level care to patients wherever they live using video-conferencing technology to train, advise, and support health care providers. Maricopa County Department of Public Health (MCDPH) intends to subcontract with a qualified Contractor to develop and implement a program based on the Project ECHO model for health care on subjects related to recovery from post-COVID-19 conditions. The RFP was released on November 16, 2022, and responses are due by 2:00pm on December 12, 2022. All responses will be reviewed by a Public Health Evaluation Committee utilizing evaluation criteria included in the RFP. A subcontract will be offered to the Respondent deemed to be the most advantageous to Maricopa County with an anticipated contract start date of January 25, 2023, through May 31, 2024. The total amount of initial funding available for this project is $500,000 and funding is being provided by the Centers for Disease Control’s (CDC) COVID-19 Health Improvement Plan grant. Should the funding source cease and alternate funding not be available, Section 4.0 of the Contract, Availability of Funds, will apply allowing for a change or termination of the agreement. If additional funds become available, any award resulting from this solicitation may be amended and/or extended based on the need for additional services to complement the existing scope of work. If additional funds become available, any award resulting from this solicitation may be amended and/or extended based on the need for continuous services as well as satisfactory Contractor performance. Any additional deliverables or changes to the scope of work and/or compensation amounts, shall be subject to negotiation and approved by all parties. (C-86-23-076-X-00)
