35. PH RFP 230086 CONTRACTS FOR COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA/CHA) 2023 SURVEY OUTREACH, ENGAGEMENT, AND COLLECTION Pursuant to MC1-329 of the Maricopa County Procurement Code, authorize the Chairman of the Board of Supervisors to execute one or more Public Health Request for Proposal (PH RFP) contracts, in a form approved by the County Attorney’s Office, to one or more successful bidders in response to PH RFP 230086 Community Health Needs Assessment (CHNA/CHA) 2023 Survey Outreach, Engagement, and Collection. Successful respondents will be awarded funds to conduct Community Health Needs Assessments to get a better understanding the impact of COVID-19 has had on Maricopa County community regions, service providers, and individual residents and the data collection will aid in identifying priority health areas and barriers in Maricopa County and the target service areas for local hospitals and Federally Qualified Health Centers (FQHC). The RFP was released on December 7, 2022 and responses are due by 2:00pm on December 27, 2022. All responses will be reviewed by a Public Health Evaluation Committee utilizing evaluation criteria included in the RFP and select those most advantageous to Maricopa County. The contract will be for six (6) months beginning January 1, 2023 through June 30, 2023. The total amount of initial funding available for this project is $600,000 and funding is being provided by the Centers for Disease Control Health Disparities Grant. Should the funding source cease and alternate funding not be available, Section 4.2 of the RFP, Availability of Funds, will apply allowing for a change or termination of the agreement. If additional funds become available, any award resulting from this solicitation may be amended and/or extended based on the need for additional services to complement the existing scope of work .
