42. APPLY FOR GRANT FUNDING OPPORTUNITY FROM VITALYST HEALTH FOUNDATION Approve MCDPH to apply for an anticipated funding opportunity from the Vitalyst Health Foundation Medical Assistance Grant. The term date of this g rant will be July 1, 2023-June 30, 2026. The anticipated funding has yet to be determined. The estimated award will be issued June 2, 2023 with the final amount and terms. This is a competitive award, at this time, it has not been determined if it will be reoccurring. There is no cash or in-kind matching requirement. Should the grant cease, ongoing contributions are not required. This Grant deviates from county Policy A2505 and does not allow for full indirect cost reimbursement, but a maximum of 10% indirect cost reimbursement as delegated in the Grant Agreement. The indirect rate for (FY23) is 17.24%. The grant award is not a mandated function but provides a benefit to the citizens in Maricopa County by supporting the expansion of the SNAP-Ed Commun ity Health Worker (CHW) Program and train CHW’s to serve the public and increase community capacity. Funding for this grant will be provided Vitalyst Health Foundation and will not affect the County’s general fund. (C-86-23-113-X-00)
