97. PARTNERSHIP AGREEMENT BETWEEN MARICOPA COUNTY PUBLIC HEALTH AND LAVEEN ELEMENTARY SCHOOL DISTRICT FOR IMMUNIZATION PROGRAM Approve a Partnership Agreement (PA) between Maricopa County, through its Department of Public Health (MCDPH), and partner school district, Laveen Elementary School District. The purpose of this Agreement is to assure the continued provision and administration of all age-appropriate for children who are uninsured and insured vaccines, seasonal influenza, and COVID-19 vaccines and for adults who are uninsured and insured. The Term of the Agreement become effective on the date both parties have signed and automatically renews annually from this date. This PA is non-financial, and the Term of this Agreement is from May 1, 2023, and ends May 31, 2026. (C-86-23-171-X-00)
