75. IGA WITH PHOENIX INDIAN MEDICAL CENTER FOR LIFE SAVING MEDICAL AND SUPPORT SERVICES Approve Intergovernmental Agreement (IGA) for the Ryan White Part A HIV Care and Services Program between Phoenix Indian Medical Center (PIMC) and Maricopa County by and through its Department of Public Health (MCDPH) to provide grant funding for HIV Care and Services. The purpose of this grant is to provide lifesaving medical and support services to people living with HIV/AIDS. PIMC will provide client-centered activities designed to improve health outcomes, such as assessment of service needs, development and updating of an individualized care plan, coordinated access to medical care and support services, continuous client monitoring, treatment adherence counseling, and occasionally assistance in accessing public and private benefits for which the client may be eligible, as well as supporting diagnostic and therapeutic services. The not-to-exceed amount is $150,000. The contract term is October 01, 2024, through September 30, 2027. (C-86-25-020-X-00)
