94. AMENDMENTS TO CONTRACTS WITH COMMUNITY HEALTH NEEDS ASSESSMENT PARTNERS Approve Amendments for service contracts with ten (10) Community Health Needs Assessment Partners (CHNA) and Maricopa County by and through its Department of Public Health (MCDPH). The contracts are hereby amended as follows; Adelante Healthcare: I. For good and valuable consideration acknowledged by the Parties, the above-named Contract is hereby amended to extend contract to September 30, 2026. Section I, General Provisions, Section 16 – Right to Extend Contract, the Contract may be extended by mutual written and signed agreement of the Parties for additional terms up to three-years each. II. All other terms and conditions of the original Contract shall remain in full force and effect. Banner Health: I. For good and valuable consideration acknowledged by the Parties, the above-named Contract is hereby amended to extend contract to September 30, 2026. Section I, General Provisions, Section 16 – Right to Extend Contract, the Contract may be extended by mutual written and signed agreement of the Parties for additional terms up to three-years each. II. All other terms and conditions of the original Contract shall remain in full force and effect. City of Hope: I. For good and valuable consideration acknowledged by the Parties, the above-named Contract is hereby amended to extend contract to September 30, 2026. Section I, General Provisions, Section 16 – Right to Extend Contract, the Contract may be extended by mutual written and signed agreement of the Parties for additional terms up to three-years each. II. All other terms and conditions of the original Contract shall remain in full force and effect. Circle the City: I. For good and valuable consideration acknowledged by the Parties, the above-named Contract is hereby amended to extend contract to September 30, 2026. Section I, General Provisions, Section 16 – Right to Extend Contract, the Contract may be extended by mutual written and signed agreement of the Parties for additional terms up to three-years each. For good and valuable consideration acknowledged by the Parties, the above-named Contract is hereby amended to extend contract to September 30, 2026.
