42. AMENDMENT TO LEASE AGREEMENT WITH SANTOS C. NIETO, SR. FOR A PUBLIC HEALTH WIC CLINIC Approve and execute Amendment No. 3 to Lease Agreement No. L-7225 between Santos C. Nieto, Sr., and Maricopa County for 1,920 square feet of office and clinic space located at 9206 South Avenida Del Yaqui, Guadalupe, AZ. The Lease rental rate will be at a flat rate of $13.83 per square foot or an annual cost of $26,553.60 plus rental tax. The term of the Lease shall be for three years commencing March 1, 2011 and terminating February 28, 2014. (C-86-96-072-4-03)

57. AMENDMENT TO LEASE AGREEMENT WITH SANTOS C. NIETO, SR. FOR A PUBLIC HEALTH WIC CLINIC Approve and execute Amendment No. 4 to Lease Agreement No. L-7225 Between Santos C. Nieto, Sr. and Maricopa County for 1,920 square feet of office and clinic space located at 9206 South Avenida Del Yaqui, Guadalupe, AZ. The Lease rental term is hereby extended for eight (8) months, commencing March 1, 2014 and expiring October 31, 2014, at a flat rate of $13.83 per square foot or $17,702.40, plus rental tax. 87. LEASE TERMINATION WITH SANTOS C. NIETO, SR. FOR PUBLIC HEALTH WIC FACILITY Approve the termination of Lease No. L-7225 with Santos C. Nieto, for the Public Health WIC facility at 9206 South Avenida del Yaqui, Guadalupe, AZ and authorize Real Estate Services staff to give formal notice to the Lessor of the lease termination. Lease L-7225 allows the County the option to terminate the agreement, without penalty, upon 90-day written notice to the Lessor. The termination will be effective on, or before September 30, 2014. Amendment No. 4 was a lease term for March 1, 2014 through October 31, 2014. Termination of this lease agreement is necessary due to federal and state funding cuts to the program. Also, an alternate clinic location within the geographic area will be available for WIC client families to receive WIC services. (C-86-96-072-4-05)
