ADMINISTRATIVE CORRECTION TO ACTION WITH THE CAZARES FAMILY INVESTMENTS, LLC FOR WIC CLINIC Approve an administrative correction to the action taken on September 1, 2010 (C-86-98-009-4-03) whereby the Board of Supervisors approved the lease agreement (No. L-7249 ) with Cazares Family Investments, LLC in which the agenda action requested incorrectly listed the square footage as 4,271. This administrative correction will correct the square foot notation from 4,271 to 4,371. All terms and conditions remain the same and in full force and effect. (C-86-98-009-4-04)

AMENDMENT TO LEASE L-7249 WITH THE CAZARES FAMILY INVESTMENTS, LLC FOR WIC LEASE Approve and authorize the Chairman to execute Amendment No. 4 to Lease Agreement L-7249 with Cazares Family Investments, LLC.as Licensor, for office and clinic space located at 438 East Southern Avenue, Phoenix, AZ, for the continued administration of the WIC Program for participating residents in the area. The term of the license will begin October 1, 2013 and expire on September 30, 2014. This lease provides space for 4,371 square feet of office and clinic space. The rate of $16.00 per square foot will remain the same for the new lease term. (C-86-98-009-4-05)

ADMINISTRATIVE CORRECTION TO AMENDMENT TO LEASE WITH THE CAZARES FAMILY INVESTMENTS, LLC FOR WIC LEASE Approve Administrative Correction on action taken, September 25, 2013 under (C-86-98-009-4-05) to approve and authorize the Chairman to execute Amendment No. 4 to Lease Agreement L-7249 with Cazares Family Investments, LLC. as Licensor, for office and clinic space located at 438 East Southern Avenue, Phoenix, AZ, for the continued administration of the WIC Program for participating residents in the area. The Administrative Correction, in the action language only, is to change the date notated in the indicated term of the license to expire as September 30, 2014 to September 30, 2016. This lease provides space for 4,371 square feet of office and clinic space. The rate of $16.00 per square foot will remain the same for the new lease term. (C-86-98-009-4-06)
