AMENDMENT TO LEASE AGREEMENT WITH BOCK FAMILY TRUST, HENRY & CLAIRE BOCK TRUSTEE FOR WIC CLINIC Approve Amendment No. 7 to Lease Agreement No. L-7142 with Bock Family Trust, Henry & Claire Bock Trustee, Lessor, for 2,976 square feet of WIC office/clinic space at 9100 N. Central Avenue, Phoenix, AZ. The lease term shall commence October 1, 2010 and terminate September 30, 2013. The base rent is a flat rate of $15.42 square foot or an annual payment of $45,895.87 plus rental tax. All other terms and conditions of the current Lease Agreement remain in full force and effect. This Amendment to Lease Agreement was approved by the County Facility Review Committee on August 26, 2010. (C-86-98-013-4-07)

TERMINATION OF LEASE AGREEMENT L-7142 WITH BOCK FAMILY TRUST, HENRY AND CLAIRE BOCK FOR MARICOPA COUNTY Approve and authorize the early termination of Lease L-7142 on or about August 25, 2011 and direct the staff of Real Estate Services to give written notice of the subject lease termination to the Lessor, Bock Family Trust, Henry and Claire Bock Trustee. Early termination was endorsed by the County Facility Review Committee on March 24, 2011. (C-86-98-013-4-08)
