54. AMENDMENT TO OFFICE SPACE LEASE WITH CENTRAL ROOSEVELT I, LLC Approve and execute Amendment Number 8 to the full service Lease No. L-7237 with Central Roosevelt I, LLC located at 1001 N. Central Ave., Phoenix, AZ. To help consolidate the space requirements of the Maricopa County Environmental Services Department and in order to facilitate office space requirements for the Maricopa County Air Quality Department, the Environmental Services Department is vacating 2,411 square feet of office space on the First Floor of the 1001 N. Central Avenue building and occupy 5,156 additional square feet of office space on the Forth Floor resulting in a revised leased premises of 35,794 rentable square feet of leased space. The rental expenses are amended to the following: 9-1-09 to 6-30-10 $559,281.25 plus rental tax, 7-1-10 to 6-30-11 $689,034.50 plus rental tax, and 7-1-11 to 8-31-11 $114,839.08 plus rental tax. All other terms and conditions of the current Lease Agreement remain the same and in full force. This item was endorsed by the Facilities Review Committee at their May 28th Committee meeting. This lease agreement may be canceled with a 90-day prior written notice. (C-88-97-024-4-08)

AMENDMENT TO LEASE WITH CENTRAL ROOSEVELT I, LLC FOR THE ENVIRONMENTAL SERVICES DEPARTMENT LEASED SPACE Approve and execute Amendment No. 09 to Lease No. L-7237 with Central Roosevelt I, LLC for the property located at 1001 North Central Avenue, Phoenix, AZ. The purpose of this amendment is to increase the leased space by an additional 2,662 square feet for the Environmental Services Department. This amendment is effective from the date of execution by the Maricopa County Board of Supervisors to August 31, 2011. The FY11 lease cost increases by $29,892 plus tax and FY12 increases by $51,243 plus tax. This lease amendment was heard and approved by the Facilities Review Committee on October 28, 2010. AMENDMENT TO LEASE AGREEMENT L-7237 WITH CENTRAL ROOSEVELT I, LLC Approve and execute Amendment No. 10 to Lease No. L7237 with Central Roosevelt I, LLC for the property located at 1001 North Central Avenue, Phoenix, Arizona. The purpose of this amendment is the renewal of lease for 38,456 square feet of office space. This amendment is effective from September 1, 2011 to August 31, 2016. Amendment No. 10 of Lease Agreement L-7237 extends the lease term for 5 additional years commencing September 1, 2011, and terminating August 31, 2016. Amendment No. 10 will adjust the rental rate downward to $17.00 per rentable square foot (plus rental tax) for the first two lease years of the renewal term. The rental rate will increase to $17.50 per rentable square foot for lease years three and four of the renewal term; and the rental rate will increase to $18.00 per rentable square foot for the fifth year of the renewal term. EARLY TERMINATION OF LEASE L-7210 Approve and authorize the early termination of Lease L-7210 on, or about December 31, 2011, and direct the staff of Real Estate Services to give written notice of the subject lease termination to the Lessor, 3101 East Shea Blvd., L.L.P., as successor in interest to Earl Capital Management, The current Environmental Services North Valley facility is located at 3101 E. Shea Blvd., Phoenix, AZ. The county leases approximately 3,800 square feet of office space at the subject site under lease agreement L-7210. The current leased space is no longer suitable for Environmental Services’ use. A replacement facility has been identified as the county-owned facility located at 15023 N. 75th Street, Scottsdale, AZ. (C-88-97-024-4-11)

28. AMENDMENT TO LEASE AGREEMENT WITH 1001 CENTRAL PARTNERS, LLC Approve and execute Amendment No. 12 to Lease No. L-7237 with 1001 Central Partners, LLC as successor in interest to Central Roosevelt I, LLC, and Maricopa County for leased space at 1001 N. Central Avenue, Phoenix, AZ. The purpose of this amendment is to extend the lease term through June 30, 2018. In addition, the amendment will eliminate the LESSOR’s right to terminate the agreement early for convenience. This amendment is effective from March 26, 2014 to June 30, 2018. This Amendment No. 12 will extend the lease term through June 30, 2018. In addition, the amendment will eliminate the LESSOR’s right to terminate the agreement early for convenience. The rental rates during the first year of the term extension will be $18.00 per rental square feet (plus taxes) and $18.50/s.f. (plus tax) for the subsequent, partial lease-year. (C-88-97-024-4-13)

37. ADMINISTRATIVE CHANGE TO RECOGNIZE NEW LESSOR UNDER LEASE L-7237 Approve an administrative change to Lease Agreement No. L-7237 to recognize 1001 N. Central Holdings, LLC as the new property owner and Lessor with regards to the Environmental Services office space at 1001 N. Central Avenue in Phoenix, AZ. The County recently received formal notice that 1001 N Central Holdings, LLC is the new property owner and Lessor with regards to Lease Agreement L-7237 for Environmental Services office space at 1001 N. Central Avenue in Phoenix, AZ. Effective September 3, 2014, all rent payments will be made to 1001 N. Central Holdings, LLC. 41. AMENDMENT TO LEASE AGREEMENT WITH 1001 N. CENTRAL HOLDINGS, LLC Approve and authorize the Chairman to execute Amendment No. 13 to Lease No. L-7237 with 1001 N. Central Holdings, LLC as successor in interest to 1001 Central Partners, LLC and Central Roosevelt I, LLC, and Maricopa County for leased space at 1001 N. Central Avenue, Phoenix, AZ. This Amendment No. 13 to Lease Agreement L-7237 authorizes the County to make modifications to the electrical and mechanical systems at the leased Premises. The mechanic and electrical improvements are part of the County-wide OET Refresh Project and the subject improvements support the County’s voice-data system at this leased location. The County will be responsible for any increase in the cost of electrical services resulting from the subject modifications. (C-88-97-024-4-15)
