47. LEASE AGREEMENT WITH MARICOPA INTEGRATED HEALTH SYSTEMS FOR AVONDALE WIC SITE Approve and execute Lease No. G-30122 (C-90-10-007-1) with Maricopa County Special Health Care District d.b.a. Maricopa Integrated Health System (MIHS), Lessor, for 2,619 square feet (sf) of office space location at the Avondale Family Health Center (FHC) located at 950 East Van Buren Street, Avondale, AZ 85323. The rental rate is $17.00 sf or an annual rate of $44,523 plus rental tax. The initial term of this lease shall be three years and shall commence upon execution by Lessee and Lessor with no provision for “hold over”. At the end of the initial term, this lease shall automatically renew for seven additional one year periods for a total of ten years, unless terminated in accordance with paragraph 8 of this lease. The purpose of the lease is to conduct the business of the Women and Infant Children Program administered through the Arizona Department of Health Services. (C-86-10-106-1-00)
