57. CHANGE ORDER TO CONTRACT WITH M. R. TANNER DEVELOPMENT & CONSTRUCTION, INC. FOR THE SUN CITY WEST MILL & RESURFACE PROJECT, PHASE 2 Approve Change Order No. 3 to Contract No. 2011-007 with M.R. Tanner Development & Construction, Inc., in an amount of $1,432,210.83 for the Sun City West Mill & Resurface Project, Phase 2, Pavement Preservation (PAVE) Sub-Project No. T377. This change order provides compensation to the contractor for the following items of extra work that were performed over the course of the project. With the exception of two items, the extra work was performed under existing contract bid items and the change order is therefore for Bid Item Overruns. The cost for the two new items was negotiated with the contractor or is based on Actual Cost Work per MAG 109.5 Specification. Many of these items resulted from changed conditions. 1) Additional Subgrade Preparation; 2) Additional Soil Cement Base Course; 3) Additional Portland Cement for Soil Cement; 4) Additional AC Pavement; 5) Additional 4” Temporary White Paint Stripe; 6) Additional 4” White Permanent Thermoplastic Stripe; 7) Additional Thermoplastic Arrow; 8) Additional Valve Frame & Cover Grade Adjustments; 9) Additional Manhole Frame & Cover Grade Adjustments; 10) Additional Survey Markers; 11) Additional Reflectorized Raised Pavement Markers; 12) Loop Detector Installation; 13) Utility Hardware Replacement. This change order will pay all direct costs associated with each item and is full and just compensation for each item. The scope of work for each item is described in detail in Attachment "A" to Change Order No. 3. An Administrative Time Extension to August 30, 2011 is included to process this change order and final payment. Supervisory District No. 4 (C-91-12-033-5-00)
