52. AMENDMENT TO IGA WITH VALLEYWISE HEALTH, FKA MARICOPA INTEGRATED HEALTH SYSTEM (MIHS) Approve Amendment 2 to the IGA between Valleywise Health and Maricopa County by and through its Department of Public Health. This Amendment shall extend the Expiration date three (3) additional years through February 28, 2025. In addition, it revises the Agreement amount to $2,000,000 annually for a cumulative not-to-exceed (NTE) amount of $6,000,000 over the next three (3) years, bringing the total cumulative NTE to $15,000,000 over the life of the IGA. Section I, General Provision, 5.0 Payment of Method has also been amended to include language regarding monitoring all subrecipients’ income and to meet grantor requirements. In addition, Section III, Scope of Work, 1.0 Service Category(s) has been amended to provide clear instructions to program services. The purpose of this IGA is to provide medically necessary health care and support services for low-income individuals with HIV infection who do not have sufficient health care coverage or financial resources for coping with HIV/AIDS. RESCIND AMENDMENT AND APPROVE IGA WITH VALLEYWISE HEALTH Rescind approval of Amendment 2 to the IGA between Valleywise Health and Maricopa County by and through its Department of Public Health action item C-95-19-005-3-02 approved on March 9, 2022. In addition, approve the new IGA between Maricopa County Department of Public Health, Ryan White Program and Maricopa County Special Health Care District, DBA Valleywise Health. This new IGA is retroactive, beginning March 1, 2022 thru February 28, 2025. The IGA has a not-to-exceed amount of $2,000,000 annually, for a total of $6,000,000 over the term of the IGA. The purpose of this IGA is to provide medically necessary health care and support services for low-income individuals with HIV infection who do not have sufficient health care coverage or financial resources for coping with HIV/AIDS.
